I have had that happen several times. One time I forgot to block the hole in the bottom of a flower pot and a molly tried to swim through the little hole. And Simon, one of my bettas was constantly wedging himself between the corner filter and the wall of the tank.

I had a DG jam himself TWICE...in a chimney on a stilted house aquarium decor... the first time I was in shock and was worried and weonderec how he even got into the chimney... the second time even though it wax tough getting him out I laughed at his stupidity tho dfo the same thing twice.

You might want to check the amount of current in the betta's tank.

I have since changed his filter from the corner filter to a small HOB that barely trickles the water back into his tank.

I had a longfin albino Danio Wedge himself into a hole in a rock and get completely stuck I had to break the rock with a pik to get him out he did not make it.

Freaking kamikaze fish I also had a 7-8inch orange Oscar jump out of the gap of the back top of the tank, found him a few days later after we noticed him missing.
